IndicationTreatment of canine congestive heart failure (CHF) originating from dilated cardiomyopathy (DCM) or valvular insufficiency (mitral and/or tricuspid regurgitation). Treatment of preclinical DCM in large breed dogs, to prolong the time to the onset of CHF or death. Treatment of dogs with cardiomegaly secondary to preclinical myxomatous mitral valve disease, to reduce heart size and increase survival time.
ContraindicationsThis product is contraindicated in cases of hypertrophic cardiomyopathy or clinical conditions where an augmentation of cardiac output is not recommended for functional or anatomical reasons (e.g. aortic stenosis).
ActionsPharmacology Pimobendan exerts its stimulatory myocardial effect by a dual mechanism of action; increase in calcium sensitivity of cardiac myofilaments, and inhibition of phosphodiesterase (type III). It also exhibits a vasodilating action through an inhibitory action on phosphodiesterase III activity.Pharmacokinetics Following oral administration of Vetmedin capsules the absolute bioavailability of the active principle is 60 to 63%. The mean plasma protein binding is 93%. The plasma elimination half-life of pimobendan is approximately 30 minutes, and the main active metabolite elimination half-life is approximately two hours. Almost the entire dose is eliminated via the faeces.
PrecautionsA moderate positive chronotropic effect and vomiting may occur in rare cases. However, these effects are dose dependent and can be avoided by reducing the dose in those cases.In studies with rats and rabbits, pimobendan had no effect on fertility, and embryotoxic effects only occurred at maternotoxic doses. In rat experiments it has been shown that pimobendan is excreted into milk. Vetmedin capsules should only be administered to pregnant and lactating bitches if the expected therapeutic benefits outweigh the potential risk.First Aid If poisoning occurs, contact a doctor or Poisons Information Centre. Dosage and AdministrationAdminister orally at a dose range of pimobendan 0.1 to 0.3 mg/kg twice daily with an ideal dose of 0.25 mg/kg twice daily. Each dose should be given approximately one hour before feeding.Daily dosage guide. < 10 kg bodyweight. Use 1.25 mg capsules: 1 each morning and 1 each evening (total daily dose of 2.5 mg). 10 to 20 kg. Use 2.5 mg capsules: 1 each morning and 1 each evening (total daily dose of 5 mg). 21 to 40 kg. Use 5 mg capsules: 1 each morning and 1 each evening (total daily dose of 10 mg). 41 to 60 kg. Use 5 mg capsules: 2 each morning and 2 each evening (total daily dose of 20 mg). >60 kg. Use 5 mg capsules: 3 each morning and 3 each evening (total daily dose of 30 mg).Vetmedin capsules may be combined with a diuretic treatment such as frusemide.
